If the CTPA had been done the same day, this have happened? I'm not saying that this is what happens where I work, but perhaps it should. Then, with the negative result, the patient would not have had to take Tinzaperin unecessarily.

For a number of reasons, we've recently decided to stop doing VQ scans routinely, and do CTPAs unless contraindicated. I could discuss this further if you like, but the protocol is on my website. While I agree that tests sometimes seem to get ordered indiscriminately, pathologists will tell you that undiagnosed PE is a common cause of death (references available).

None of this makes my work any easier, but it is important to see things from the other side.

Here are my experiences:

http://pengrad.blogspot.com/2006/09/digital-dictation-and-voice.html

However, some people find it works well, as you can see from the comment to my post, and the system I tried was bolted-on to a poor-quality RIS, that was not integrated to PACS. I'm still keen in principle and I'm planning to visit some places where the voice recognition is said to work properly. We will be installing a new Agfa PACS, to be integrated with an upgraded RIS, so fingers crossed.

Hello, MadRad, belated thanks for the link to this fascinating document. I just had a look at the headcount figures. I can't actually make sense of the "NHS infrastructure support" number because it is smaller than the "Admin and clerical" number....... anyway, one reason for the increase in the number of people on the management payscale (which is what this is counting) is that job definitions get, um, creatively worded, in order to be able to recruit someone halfway good in competition with the private sector, or at least other bits of the public sector (eg local authorities). There are plenty of people on the NHS management payscale would would not be considered managers outside the NHS.

Heh. That gave me a chuckle. An aircraft carrier would raise a few eyebrows at the DoH, especially if you're cheeky and name it the HMS Hewitt :)

[...] [1] I have lost count of the times I have been asked “Can I have it done quicker if I go private?”. The answer is ‘No’. It will always be ‘No’. I do not have the luxury of keeping empty slots for a PP just in case one comes along. I book cases by clinical urgency. Most scans have a 2 week wait anyway. [2] You will note I refer to Alliance as an example. There are, of course, other companies such as Lodestone, fulfilling similar roles or aiming to do so in future contracts. I am however particularly worried about Alliance. That’s another story. Please refer to the posting entitled ‘Unholy alliance‘ for further details. [3] Including advertising. shortlisting and interviews. CRB checks are currently taking two months or so and no one in their right mind would give notice until all checks are through. AfC requires 2 months notice. [...]
